All required participation forms must be submitted ONLINE before a student participates in any try-out, practice, athletic class, open gym, open weight room, athletic competition or travels with a KISD athletic team for any purpose. RankOne Link for Physical Forms and Medical History
The student is required to use the Pre-Participation - Physical Examination Form; NO OTHER Physical Examination Form can be accepted as per the University Interscholastic League. Please use the link on the right hand side to find it and print it.
For SAC Camp, current physicals are still good through July 31st.
All incoming 7th Graders must get a physical completed after May 1st for SAC Camp, which will also be good for their 23-24 School Year Athletics.
For this school year, 2023-2024, ALL students must submit new physicals to their Rank One Accounts. Physicals must be dated May 1, 2023 or later.
